Job Description
Affect improvements in the personal lives of individuals seeking care by providing professional case management services.



ESSENTIAL FUNCTIONS
• Engage individuals and families as partners in services and develop positive, hopeful, and trusting relationships.
• Manage the emotional distress experienced by clients and their families in avoiding and resolving crisis situations.
• Recognize and demonstrate sensitivity to cultural and ethnic differences.
• Help others succeed by demonstrating exceptional interpersonal skills and the ability to work with persons with severe and persistent mental illness and/or serious emotional disturbance, and the ability to react effectively in a wide variety of human service situations. Effectively manage conflict, promote change and growth, and inspire the development of all clients served.
• Build client relationships, as well as business relationships and strategic partnerships with other agencies and external resources.
• Complete all documentation and reporting requirements thoroughly and on time.
• Interact positively with other members of the agency and team, including active participation in supervision activities, team meetings, continuing education, etc.
• Regularly travel and transport clients and/or families in and around the region, and provide direct care and support as needed.

Job Description
These positions have flexible scheduling. Can work up to 27 hours per week.

Provide assistance and constructive guidance to individuals in completing daily tasks and activities. Provide training in life/social skills. May work at a specific organizational facility, an authorized community/program location and/or client home. Works under general supervision.

• Provide transportation and accompany clients to and from activities and/or appointments to provide medically necessary direct care and support, as directed.
• Assist in providing structured and constructive activities that support the client’s treatment.
• Assist clients with learning and practicing daily living and social skills.
• Assist client in completing daily living tasks as needed.
• Assist clients with problem-solving daily living situations.
• Complete all required documentation thoroughly, on time and according to agency standards.
• Provide above services on an individual basis or in a group setting as required.
• Engage individuals in a positive, constructive, respectful and hopeful manner.

Must be available from 9:00 - 4:00 Monday through Friday

or

Must be available to 4:00 p.m. to 9:00 p.m. on Saturday and Sunday
