Provide street outreach services to identify homeless individuals who may need mental health services. Identifies strengths and needs of consumers and acts to involve them in planning for desired services. Work closely with other agencies who serve the homeless to identify and serve individuals with severe and persistent mental illness.Work with landlords to develop housing resources for client. Recommend payment of client’s rent, deposit and utilities when appropriate.Familiarize clients with community resources for educational, cultural, recreational and social activities. Works with social and Rehabilitation Services, Social Security Administration and other agencies to secure entitlements for financial support. Assists consumers in accessing desired housing.Transitions consumers to ongoing case management services once housing and other needed resources are obtained.
